We utilize a program called Klara that allows us to text (HIPAA Compliantly) with our patients.  If you have a cell phone, we can text digital forms to you that you can fill out right on your phone, tablet, or computer.  If you are unable to fill your paperwork out in this way, we ask that you download and print the pdf form below, or come in a half-hour early in order to fill out your paperwork.

For new patients we have implemented a $50 no-show fee. We set aside time specifically for new patients and we want to make sure that everyone that would like to come to our practice is able to. We kindly ask that if you need to cancel your new patient appointment, you do so at least 24 hours in advance to avoid the no-show fee. This will allow us to schedule someone else that is waiting to be seen. We appreciate your understanding, and look forward to seeing you for your appointment.
